To obtain your transcript of marks or attestation of attendance:

• If you were an EMSB student within the past 5 years, please contact the school you attended directly.

OR

• If you attended a PSBGM school, please follow the procedure outlined below:
• Download a request form in PDF format in English or French
• Complete the form
• Include a copy of a photo identification document such as a driver's license or Medicare card.
• Mail to:

Request forms may also be dropped off at the EMSB Administration building on Monday to Fridays between 8:00 am to 4:00 pm.
NOTE: The request form cannot be submitted by email.
